Filing requirements may differ between chapters. Additional documents may be required for each chapter if the filing is a corporation, the case is prepared by a bankruptcy petition preparer or joint debtors have different last names. Consult the Local Bankruptcy Rules for additional information. The debtor must bring a paper copy of the petition, schedules, statements and lists to the meeting of creditors so that the trustee may use the paper copy to examine the debtor.
The form numbers listed below refer to the National Form Number (e.g. 101, 203), Director's Form Number (e.g. 2030), or the Local Bankruptcy Form Number (e.g. LBF 3015-1). A link to the National Forms and the Local Bankruptcy Forms can be found on the Rules and Forms page.
Individuals filing for bankruptcy must use forms that are numbered in the 100 series. Non-individual debtors must be represented by an attorney and use the forms that are numbered in the 200 series.
